FS Vehicle Requistion
This form is for the use of Facilities Service's Employees to request vehicles.
For any new vehicle requests this form must be filled out as completely as possible and signed by you before submitting it to Mike Fujita. Please keep in mind that just by submitting the form does not guarantee you will get the vehicle as each purchase has to be approved.
