All work related injuries and illnesses shall be reported to your Supervisor. Supervisors shall investigate the incident and complete an Employer's First Report HERE(link is external). Instructions can be found HERE(link is external).
The following incidents must be reported to 510-642-3073 immediately:
· Worker Fatality
· Inpatient Hospitalization
· Amputation
· Possible permanent disfigurement
· Injuries of three or more workers
